रोग प्रतिरोधक क्षमता में सुधार लाना।

एलर्जेन इम्युनोथिरैपी के प्रकार:

१. एस.सी.आई.टी- सब क्युटेनियस इम्युनोथिरैपी- चमड़े और मॉंस के बीच इंजेक्शन से दवा दी जाती है।

(क) कंवेंशनल, (ख) रस, (ग) अल्ट्रा रस

२. एस.एल.आई.टी- सब लिंगुअल इम्युनोथिरैपी- जीभ के नीचे रखने वाली दवा।

हर के अपने अपने फ़ायदे एवं नुक़सान संभव हैं।

एलर्जी का ईलाज

१. बचाव- जिन चीज़ों से एलर्जी हो।

एलर्जिस्ट कौन होते हैं?

एलर्जिस्ट डॉक्टर वो होते हैं, जो सिर्फ़ एलर्जी रोग का हीं उपचार करते हैं।

कुछ डॉक्टर अपनी बेसिक डॉक्टरी की पढ़ाई पूरी होने के बाद, कुछ समय तक सिर्फ़ एलर्जी रोगों का अध्ययन करते हैं, और बाद मे जाकर एलर्जी रोग विशेषग्य बन जाते हैं।

भारत वर्ष मे एलर्जी की पढ़ाई एक-दो मेडिकल कॉलेजों मे ही होती है, जैसे सी एम सी- वेल्लौर।

एलर्जी के जॉंच

१. स्कीन प्रिक टेस्ट- जॉंच से पहले बीमारी नियंत्रण मे होनी चाहिए, जॉंच को प्रभावित करने वाले विभिन्न दवाओं को कुछ समय के लिए टेस्ट से पहले बंद कर देना होता है।

२. ब्लड टेस्ट- ख़ून में ‘स्पेसिफिक आई जी ई’ का जॉंच- कभी भी जॉंच की जा सकती है।

दोनों टेस्ट की अपनी अपनी विशेषताएँ एवं कमियॉं हैं।

एक ही मरीज़ में दोनों टेस्ट की आवश्यकता पड़ सकती है, एक हीं मरीज़ मे एक साथ दोनों टेस्ट कराने पर अलग अलग रिपोर्ट आना संभव है, एक ही मरीज़ मे अलग अलग समय कराए गए टेस्ट मे अलग अलग रिपोर्ट आना संभव है, इसलिए एलर्जी टेस्ट कभी भी बिना चिकित्सक की राए लिए नही कराना चाहिए।

एलर्जी के प्रकार

एलर्जी शरीर के किसी भी अंग को प्रभावित कर सकती है।

१. चमड़े की एलर्जी-

क) एक्यूट अर्टिकेरिया

एलर्जेन क्या हैं?

एलर्जेन वो तत्व हैं जिनके कारण हमें एलर्जी की बीमारी होती है।

अलग अलग मरीज़ों को अलग अलग चीज़ों से एलर्जी हो सकती है।

प्रकार:

१. डस्ट माईट- धूल मे रहने वाले सूक्ष्म जीव

२. पॉलेन- पेड़ पौधों के पराग कण / फूलों की ख़ुशबू

३. फ़ंगस- काई

४. फ़ूड एलर्जेन- भोजन से एलर्जी

५. ड्रग- दवाओं से एलर्जी

६. इन्सेक्ट्स- कीड़ों से एलर्जी

इत्यादी।

एलर्जी क्या है?

एलर्जी एक बीमारी है जो शरीर की रोग प्रतिरोधक क्षमता के सामान्य से ज़्यादा काम करने के कारण होती है।

उदाहरण के स्वरूप, नाक में धूल जाने से सबको छींक आती है, उन्हें भी जिनको कोई बीमारी नही है, किन्तु एलर्जी के मरीज़ जिन्हें धूल से एलर्जी हो, उनकी छींक रुकने का नाम नही लेती। छींक के बाद खाँसी फिर दम फुलना शुरू हो जाता है।

Can allergy be cured

No, but you can manage it to live a normal healthy and productive life.

once your allergen is identified you can take better precautions.

suppose you are allergic to milk, you will need to forget milk for whole life. If you continue milk avoidance you will live a hassle free life. It doesn’t mean you are getting cured as re exposure to milk in future may result in recurrence of symptoms.

some patients do outgrow their allergies with time, like children develop tolerance to milk with time. But it’s impossible to tell which child will outgrow and which one will not in beginning.

Immunotherapy helps in developing resistance to a particular allergen. It’s immune system modulation which builds up immunity in general and against particular allergen, then it has a dose dependent relationship with symptoms. Your body starts resisting small amount of allergen and dose and frequency of medicines start coming down.

There are many factors which determine the outcome of immunotherapy like:

Single allergen vs multi-allergen

stage of disease at the time of starting IT

type of allergen.

Dust mites, pollens, insects show very good response while fungus immunotherapy is still in nascent phase. Food allergens are not tried, avoidance is the only measure.

we do use biological agents like Omalizumab as and when required.

Immunotherapy

We prefer SCIT, i.e. Sub Cutaneous ImmunoTherapy for most of the allergens. We also have SLIT i.e. Sub Lingual ImmunoTherapy for dust mites.

Precaution for Allergy tests

Skin prick test demands some condition to be fulfilled:

1. Disease should be under control

2. Anti allergy medicines need to be stopped for minimum 1 week

3. Different groups of medicines need to be stopped for different duration.

Ideally you should visit for consultation first, upon going through all the reports, medicines, health condition, you will get your appointment for allergy test on a particular date.

How long it takes here

Depending on your condition, previous reports available, need of new investigations, it takes 1 hour to 6 hours.

Few reports may not become available on same day, you will need to visit again for them.

Validity of a new prescription

A new prescription is valid for 21 days. You will be charged for consultancy after that.

Do you admit patients

Yes, we do admit for short duration. Mostly we provide day care facilities. We have HDU, general ward and special wards.

Patients in need of ICU are referred to nearby hospitals with ICU.

What diagnostic facilities are available in-house

1. Laboratory- conducting all blood tests, sputum tests, urine tests required in an allergy asthma COPD or other patients we see.

What is allergic sensitisation

Sensitisation is different from allergy.

sensitisation means a person has been exposed to that particular allergen. It is confirmed by either skin prick test or by presence of specific IgE in blood.

so, allergy tests are basically tests of sensitisation. To prove allergy to that particular allergen clinical correlation is necessary, either by history or challenge test.

What is atopy

Atopy is genetic predisposition to form increased amount of IgE to a stimulus.

What is allergy

Allergy is hyperactivity of our immune system. It occurs in atopic individuals upon 2nd exposure of the allergen to which a person was sensitised previously.

It occurs due to tilt of our immune system in favour of Th2 mechanism compared to Th1.

For example, exposure of dust results in sneezing or cold in any healthy individual, but it leads to prolonged cold in a person allergic to dust. Depending on level of allergy the cold progresses to cough and shortness of breath. So it’s hyper response to normal immune mechanism.

Who is an allergist?

Yes.

The Amended HIPAA Privacy Rule states only that you must receive a Privacy Notice telling you how your personal health information will be used and disclosed. Section 164.520(c) (2) (i) (A).
